Diabetes is described as a complex chronic condition, and is generally with someone their whole life, requiring daily self-care through diet, medication and general health.
There are three main types of diabetes: type 1, type 2 and gestational diabetes. For more information visit the Diabetes Australia website.
Our Doctors and Nurses work closely with patients to establish goals and care plans suited to each individual.
We have access to: Diabetes Educators, Dieticians, Podiatrists, Exercise Physiologists, and Physiotherapists, where patients are able to focus on their weight and overall fitness in a controlled environment.
Our practice also encourages diabetic patients to have regular reviews every 3-6 months (depending on control), along with their 12-monthly Annual Diabetes review, where bloods and other parameters will be reviewed.
Referrals to access Allied Health Providers such as Podiatrists and Diabetic Educators can be organised at these reviews.
